QUICK START GUIDE - Form Section - Question List

  • Checklists are aids that help you review configuration settings and follow best practices prior to testing. Be sure to test the functionality fully in your Pilot portal before releasing to end users.
  • The Form Section Quick Start Guide covers the creation of the available form sections to be used in a Performance review task.

GO TO Admin > Tools > Performance Management > Form Sections > Create Review Section

 

BEST PRACTICES FOR form sections

  1. define review section
  1. Name
  • Name your Question List something that will be easily identifiable when bringing into a Performance Task.
  • Select Question List for the Section Type.
  1. Details

  • Directions: Enter directions that will appear to end users at the top of the performance review section within the Performance Task. HTML can be used to highlight important instructions.
  • Leverage tags to pull in relevant information.
  • Categories should be used to organize multiple sections for easy identification for future use.

 

  1. Rating
  1. Rating Scale
  • The default rating scale is automatically pulled into each new performance review, but can be modified at the section level. Editing the rating scale here will only change the rating scale in the review for the Goal Rating Section. To modify click the Edit button.
  • You can choose to Display Rating Scale or Hide Rating Scale from users.
  • Display Scores to End Users option allows for the numeric scores to display along with the text rating.
  1. Rating Method
  • Decide if you want the reviewer to rate by the Drop-Down Rating or the Star Rating Scale.
  • The Rating Method section is only active when the score interval for the rating scale is 1.0. If a score interval other than 1.0 is selected for the section rating scale, the Rating Method options are disabled and a text input rating option is implemented for the section.
  • Once a performance review section is saved, any change to the Rating Method causes the section template to create a new version.

  • Click Add Custom Fields to select the performance review custom fields to be added to this section of the review.
  • The Custom Fields section is only available if the administrator creating the form section has availability to at least one custom field.
  1. Placement
  • Once custom fields are added to the review form section, you can reorder them by dragging and dropping them into your defined order.
  • Select the Required option if you want to require users to complete the custom field. If left unchecked, the field will be optional

  • Here you can decide if you want the question numbers to display on the performance review
  • You can also decide if you would like the question weights to display in the printed version of the review.

Add question from your question bank to pull into this form section. A new window will pop up when the +? is clicked.

  1. Question Bank
  • Click the + next to the question you would like to add to your question list.
  • You can search for questions you have previously created using the search bar at the top of the Question Bank window.
  • For new questions, click Create New Question.
    • Type your question in the question box.
    • Select your response type.
      • Rating Scale responses will utilize the rating scale previously set within the Question List configuration. Rating scales can include a comment box which can be a required field. Specific directions for comments can be entered as well.
      • Text Only responses utilize an open text box where end users can enter in their own response.
    • Select a category to assist in organizing your questions for future use. New categories can be created at this time.
    • Add text for suggested comments if you would like to provide your end users with suggested responses when answering this question.
  • Click Done once all questions have been selected.
